feat(plugin-mcp): add MCP tool annotations

Add MCP tool annotations to built-in and custom tools so clients can understand titles and behavior hints like read-only, destructive, idempotent, and open-world. Built-in annotations can be overridden through config, and annotations.title is now also used as the internal display label for tools, which also bridges the gap between internal and external API for labels (custom tools were not able to define a label).
